Informed Consent & Client Agreement

This document is completed and signed as part of onboarding, after your free discovery call - it isn't a form you fill in on this website. It's published here so you can read it in full beforehand.

This document is for adults aged 18 and over. If you are under 18, a parent or guardian must complete a separate consent form before any sessions can take place. Please read each section carefully before signing.

Transforming Thoughts Ltd provides NLP-informed coaching and non-medical hypnotherapy for personal development purposes. These services are educational and complementary in nature. They are not a substitute for medical diagnosis, psychiatric treatment, or regulated mental health care.

The practitioner is a Member of the International Alliance of Professional Complementary Practitioners (IAPCP) and operates within their published Code of Conduct, a copy of which is available on request.

Sessions are delivered online; in exceptional circumstances, and by prior agreement, a session may be held in person. A complimentary discovery call is available prior to booking so you can ask questions and assess suitability before committing.

By signing this form, you confirm that you understand:

  • These services are not medical or psychological treatment and are not regulated by a statutory health body.
  • No guarantees are made regarding outcomes. Your progress depends on your engagement, commitment, and individual circumstances.
  • The practitioner will not advise you to stop any medication or treatment prescribed by a medical professional. If you have concerns about an existing prescription, you should raise this with the prescribing clinician.
  • The practitioner reserves the right to decline, pause, or terminate services where, in their professional judgment, the service is unsuitable or safe continuation is not possible.
  • If the practitioner believes a referral to another professional would better serve your needs, this will be discussed with you openly and without pressure.

You agree to:

  • Attend sessions voluntarily and take responsibility for your own wellbeing during and between sessions.
  • Disclose any relevant health conditions, medications, or circumstances that may affect your participation - before or during the course of your sessions.
  • For online sessions: ensure a stable internet connection, functioning audio and video, and a quiet, private environment. Technical issues on your side may be treated as a cancellation in accordance with the Terms & Conditions.
  • Where a session is held in person by agreement: arrange your own travel and arrive punctually. Late arrival may result in a shortened session.
  • Not record any session, in whole or in part, without prior written consent from the practitioner.

The following is a summary. The full Terms & Conditions govern in the event of any dispute.

  • Sessions are confirmed on receipt of a booking form and full payment in advance.
  • You may reschedule up to 48 hours before a session using your booking link or by emailing support@transformingthoughts.com.
  • No-shows, rescheduling requests made within 48 hours, and cancellations at any time are charged at the full session rate. Payments are non-refundable.
  • Pre-paid sessions or packages must be used within one month of the originally scheduled session unless otherwise agreed in writing.
  • If the practitioner cancels due to illness or unforeseen circumstances, the session will be rescheduled at no additional charge. No refund is due where an alternative appointment is offered.

Everything you share in sessions is treated as confidential. Your information will not be disclosed to any third party without your express written consent, except in the following circumstances:

  • Where you give explicit consent - for example, for your practitioner to liaise with your GP or another healthcare professional.
  • Where there is a serious and credible risk of harm to you or to another person.
  • Where required by law, court order, or direction from an authority with legal power to compel disclosure.

Where disclosure is made without your prior consent, this is documented in writing, including the reason, recipient, date, and information disclosed. Where practicable, you will be informed beforehand.

Your personal data is processed in accordance with UK GDPR and our full Privacy Policy. In summary: your data is held securely, used only for the purpose of providing and administering your care, and retained for seven years from your last appointment. For any data-related queries, contact privacy@transformingthoughts.com.

If you are unhappy with any aspect of your experience, please raise this in writing in the first instance: support@transformingthoughts.com. We will acknowledge your complaint promptly and work to resolve it fairly. If you remain dissatisfied, you may escalate your concern to the IAPCP or, for data protection matters, to the Information Commissioner's Office.

Before sessions begin, you'll be asked to confirm each of the following, typically by initialling or signing:

  • I have read and understood the nature of the services provided and confirm they are suitable for my current needs.
  • I understand these services are not medical or mental health treatment and are not a substitute for professional medical advice.
  • I have disclosed all health conditions, medications, and relevant circumstances that may affect my participation.
  • I understand and accept the cancellation, payment, and rescheduling terms as summarised above and set out in the full Terms & Conditions.
  • I understand the limits of confidentiality as described in Section 5.
  • I confirm I am 18 years of age or older (if under 18, a parent or guardian must sign a separate consent form).
  • I consent to my personal data being processed in accordance with the Privacy Policy.
  • I understand I may withdraw from sessions at any time, and that doing so does not entitle me to a refund for sessions already paid for.

This agreement is signed as part of the onboarding process, following your discovery call, alongside the practitioner's own signature and the date received.
